sheet from your school’s

Anaphalaxis / Asthma plans to be provided to school.
You will also need to provide the school with the prescription-labelled
medication (must have Doctors name on the label), in the original packaging with clear directions for its use.
All over the counter medications are also to have a prescription-label and must have the Doctors name on the label. The over the counter medication must be in the original packaging with clear directions for its use.
